Ä¢¹½tv Punxsutawney D2A2 Checklist for Students:

  1. Complete the Declaration of Disability form (found on our Forms web page), and submit it to D2A2.

  2. Provide supporting documentation of your disability.

  3. Once you're registered for classes at Ä¢¹½tv Punxsutawney, complete the D2A2 Scheduling Form for the upcoming semester (found on our Forms web page). You will need to complete this form every semester.

  4. Make sure that you contact the Punxsutawney administrative office (which will serve as the liaison between Ä¢¹½tv Punxsutawney and D2A2) during the first week of classes to discuss coordinating your accommodations and to pick-up the D2 A2 Student Handbook.
